APPOINTMENT REQUEST:

If you want to improve your smile or are in need of dental care, we welcome you to CONTACT US and arrange an appointment with DR. LIM and see the difference our office can make for your oral health. If it'll be your first experience, you may be required to complete and submit a NEW PATIENT FORM to our team. We will be excited to have you as a part of our dental family!

RESCHEDULE:
We understand that unforeseen situations sometimes occur, and we welcome your proactive effort in informing us in advance. We look forward to meeting your requirements and providing the dental care you deserve.

CANCELLATIONS:

Kindly inform us 24 hours ahead of time if you are unable to make it to your scheduled appointment. This way we can give the opportunity to another patient who might need it. Please remember that not canceling with notice may lead to a penalty. We value your understanding and cooperation.
